The Member Services team at Rogue Community Health is responsible for providing personal outreach to people on the Oregon Health Plan (OHP) who have been assigned by our Coordinated Care Organizations (CCOs), Jackson Care Connect (JCC), and AllCare. The team does monthly outreach to newly assigned OHP members to help them establish a Primary Care Provider, and assists members in accessing specific elements of their care plans.

At Rogue Community Health we realize that there is more to health and wellbeing than medical care, especially as it relates to fostering self-sufficiency in our community. Our Community Health Workers (CHWs) are non-clinical professionals who help our members access services related to social factors such as food, housing, transportation, personal safety and security, and behavioral health concerns. CHWs help our members assess current resources, reduce barriers to obtaining services, and offer support in skill-building to develop plans to reach goals that permanently address these issues.

Community Health Workers receive referrals from members’ Care Teams, and new members are screened to detect issues that may challenge them in getting to appointments, meeting their care plan, or ways that we may be able to support them beyond their physical health needs.
