At Rogue Community Health we realize that there is more to health and wellbeing than medical care. Our Community Health Workers (CHWs) are non-clinical professionals who help our members access services related to social factors such as:

  • Food assistance
  • Housing assistance
  • Transportation
  • Benefits assistance (SNAP/TANF/Disability)
  • Personal safety and security
  • Navigating behavioral health support
  • Addictions and recovery services
  • Childcare
  • Navigating health insurance

CHWs help our members assess current resources, reduce barriers to obtaining services, and offer support in skill-building to develop plans to reach goals that permanently address these issues.

Community Health Workers receive referrals from members’ Care Teams, and new members are screened to detect barriers that may challenge them in getting to appointments, meeting their care plan, or ways that we may be able to support them beyond their physical health needs.
